Must read in safety/reliability field
A must read for managing safety and reliability. Good amount of explanation on topics necessary for the work in safety and reliability- I find myself using sentences and phrases right from the book to teach/coach others.

A Human Factors 'must-have'.
Dr. James Reason's Managing the Risks of Organizational Accidents is a must-have for every Aviation Safety Officer or Aviation Safety Manager's professional library. Great book!

Excellent job . . .
This book does an excellent job in describing the challenges in designing managerial systems to prevent accidents in extremely complex engineering projects. If that sounds dry it is although I have I tolerance for this type of writing, the average reader may not.The book gives a variety of examples of complex engineering failures and how they happened. The author also goes into great detail as to how these failures occur despite the best efforts of engineers to prevent them.Like another reader I found it disturbing that the author did not offer any advice about how to prevent these type of accidents so I came up with my own: if the potential for an accident seems almost inevitable, then in those cases that the consequences of an accident are to horrible to contemplate .. . don't build them. That in my mind does not bode well for nuclear power.

Great book
This book was really useful for a decision analysis course. Interesting examples in the book. I would recommend this for working professionals that are interested in mitigating risks, students that are taking a decision analysis, risk management, project management, engineering ethics or other related courses, or for general readers who just want to understand what events or factors lead to organizational accidents.

Great for QA not just safety
This has to be the seminal work. It provides a lot of great information that can be applied to quality defects and why people make mistakes or don't follow procedures. The charts are some of the best parts of the books.

Excellent condition for an old book!
The condition is perfect, especially for a book made in 1997, there is no tear or scratch, coloring, or any other kind of damage to the book. I've been using it often, since it is a textbook for my master of occupational health degree. It is really worth the price.

The Classic
The classic. James Reason, inventor of the “swiss cheese” model provides comprehensive and well balanced insight into prevention of safety incidents in an organization. Must read for anyone in the Health&Safety

exceptionally clear and useful
Not surprised that this book has been reprinted so many times. Even though it is now a few years old the ideas (which are almost throughout very clearly and simply expressed) remain pertinent to numerous industries and fields of activity including children's safeguarding which is my concern. Highly recommended

O mordomo não é o culpado.
Gostei muito da sensibilidade e perspicácia do autor para descrever e perceber os detalhes e nuances envolvidas em um acidente.
