does the white and black wire go to output jack?

The black wire is your ground and goes to the sleeve. The white wire is your "hot" and goes to the tip. If in doubt, test your connections with alligator clips before soldering.

Is this compatable with a Yamaha Pacifica?

Unfortunately no. It would not be an even replacement as these pickguards are made to fit more of a standard Fender stratocaster and even at that, it’s not a perfect fit. The issue would be three things. 1. Fender uses a different screw hole pattern than Yamaha. 2. The neck pocket radius is vastly different. 3. The bridge width is not compatible. However, if you have your heart set on a pickguard replacement, all is not lost. There are a few online companies who will custom make a pickguard for your model at a high quality that won’t drain your wallet. Expect to wait a while though.

Is the humbucker a 2 or a 4 wire pickup?

There are two wires on this humbucker. I would recommend using a body that has swimming pool routing. The body I'm using has the H.S.S. rout and I had to extend the single coil pickup wires. Your mileage may vary though.
