Q: does this overlay need a cover?

I put a quilted mattress pad and a fitted sheet and then sleep on it. I did not buy a special cover for it.

Q: Does it go over the sheet or under?

Hi, we have used these for years... It goes over mattress and sheets go over the gel mattress and mattress underneath. In other words, the "deep mattress" sheets are the best to cover both mattresses at once.

Q: Can head & foot be raised?

This is a gel mat that you put on top of a mattress. It is very pliable and moves/bends with the mattress.
