How long is the cable on the omnidirectional mic?

The cord on the mic looks to me to be around 5 or 6 feet long.

Depends on what you mean by "usb mic preamp." Does said pre-amp supply 48V "phantom power," or not? If yes, then the answer is, "yes." This mic is impedance-matched for 48V "phantom power." Most lavalier mics would be fried at 48V, so if you are using 48V "phantom power," then you will need an impedance-matched one like this. Some usb preamps can supply 1.5V-5V "plug-in" power (often referred to as "camera" setting) in which case you could use plain old, much cheaper 3.5mm (TRS, not TRRS) lavaliers. Does that make sense? The Zoom H4N can supply both "plug-in" power and "phantom" power. The Zoom H2N can supply "plug-in" power. It appears that "usb preamps" come in both flavors. So check the specs of YOUR preamp.

Will this mic run of 12v or does it require 48v phantom power?

Your second guess is correct. It only can be powered by 48v DC from a device like a professional mic mixer and other devices, including some professional video cameras.

Can I use 2 of these at the same time with a zoom h4n pro? If they both require 48v, does that mean i need a total of 96v?

You might want to keep in mind that the Zoom H4N also provides "plug-in" power (about 2V) so you could use much cheaper 3.5mm (TRS, not TRRS) lavalier mics. On the other hand, this MOVO 48V lavalier would work on a wider variety of mixing boards, very few of which provide "plug-in" power. Fun fact: the Zoom H2N provides "plug-in" power, but not 24/48V "phantom power."
