Will it cover around the whole leg of a medium size lady? I need to use it for chemo.

The ice pack doesn't fit around my entire leg. The ice pack is 14 inches long but my thigh is 22 inches. However the straps are plenty long if you need those to go around any body part

How long is it from the top near the neck down to the end where the elbow is?

It is 11x14 so depends on which way you secure it. We found that we could secure the Velcro in different ways to hold it in postion that was most comfortable depending on area of body used. We have used it shoulder, upper and lower back, calf and thigh so far
