Poor quality control.
If you want a set of questions that regurgitate NICE guidance without considering the context in the question stem - go for it. Even then, you'll be left with contradictory explanations, and question stems (that by apparent misprint) leave out all of the intended investigations. Leagues apart from the (free) official practice questions on the MRCP website.
