Best brace!
I have severe de Quervains and this brace is the only thing that gives me relief from the pain. I am able to sleep with it on throughout the night without pain and wear it throughout the day without pain. This brace is NOT comfortable but it does what it’s suppose to do. This brace does not cure my de Quervains but it relieves my pain. I have bought about 10 different splints before finding this one and this is by far the best one I have found. You can’t beat the price. I have had shots in my tendon that did not work and for this brace to relieve my pain is a blessing. It definitely stabilizes the thumb and is very durable and well made. I would definitely recommended this brace to anyone. I will buy this brace over and over again.

Works
This is the only brace I've tried that really immobilizes my thumb. The metal splint part is not very comfortable but I've bent that area on the non metal ones I tried with my thumb, so it was not immobilized and would not heal. I placed 2/3" thick soft foam padding the entire length of the brace, about 1.5" wide where the metal is and a small piece at the v between my thumb and index finger. It is much more comfortable and very snug, which helps keep it immobile. Nearly healed after a month with this brace so it does work very well. The tenosynovitis has been ongoing for 8 months or so, wish i had found this earlier.

Metal supporting thumb can and should be adjusted
When I first received this several days ago and put it on, I was screaming inside, disappointed and frustrated because I had been expecting it to be as comfortable as the Aircast without the thumb spica for my right wrist I had used. This version was extremely uncomfortable when I first put it on, around the thumb where the metal was. However I did discover that the metal stay keeping the thumb in place can be removed (as can the volar stay on the palm side of the splint) and you can then bend it pretty easily and make it comfortable. I suppose one might need help with this if they already have pain in their wrist, but it is done easily enough, so I don’t imagine it would be a big deal to get help with this task. It isn’t made so that the thumb part is going to fit most people right out of the package, so I expect that this is a necessary step for most people if they want to be able to wear this. I actually bought another wrist brace immediately after putting this on and before discovering that I could mold it to fit comfortably, and while I found the other brace to be comfortable and pretty supportive, the Aircast one is more supportive than any I’ve ever used (aside from custom made splints, which have their own pros and cons) and I think I prefer the Aircast now. I don’t think I really need the thumb spica and I did manage to find the size Aircast I need (large) for my left wrist without the thumb spica on a different website, as it wasn’t available on Amazon yesterday when I decided I wanted the regular one and I’m hoping it will arrive quickly enough. But this one will work until then and I encourage everyone to adjust the thumb part or it will feel very uncomfortable. I couldn’t have kept wearing it without adjusting it, but I’m even wearing it overnight now.

Definitely works
My husband was having terrible radiating thumb pain from his thumb joint through his wrist. We bought this to stabilize everything for a few weeks. It worked amazingly. He used this in combination with some light stretching of the thumb a couple of times a day. He works long days carrying heavy boxes and so it did wear out pretty quickly. The seems open up and the fabric wears through and the metal bar at the wrist starts to protrude. We had to buy another one to replace the first, and then the second started to wear out, but it DOES work. It's not super expensive, and it did solve the problem so we didn't need a doctor visit or specialty fees etc. That was nearly two years ago and he still hasn't had any relapses with the pain since. Very thankful for this product.

New to the spica thumb brace experience
This is the first wrist support spica thumb brace I purchased after a diagnosis of de Quervain Syndrome. I was told by the doc to save money and order a brace online instead of getting one from the hand rehap center. I figured that meant the braces were pretty standard, so I did my research and chose this one. Pros: (1) It arrived within 2 days. (2) The size I chose was correct. (3) I liked the lightweight fabric on the top; it allowed a fair amount of breathing space for the skin. Cons: Within one hour, it was too painful to wear. I kept re-positioning it, readjusting the velcro, and eventually did some modifications. I still couldn't wear it. Here are the problems: (1) The plastic loops that the velcro went through pushed hard against the wrist bones and skin. Very uncomfortable. Eventual modification: I cut them off. (2) The velcro is so strong that I literally had to use my teeth to undo the straps. Frustrating and probably bad for my teeth. Perhaps if I didn't have arthritis in both thumbs, I could have used my other hand to undo the straps. Eventual modification: I cut them off. (3) The metal component of this brace dug deeply into the base of the thumb. I could not re-position that enough. Eventual modification: bent the metal outward with pliers -- just a tad so the pain would stop. That helped. (4) Biggest problem, however, was the positioning of the thumb. I noticed it was tucked down under the index finger which seemed unusual. With the brace off, my thumb was fine -- until I tucked it under the index finger for an extended time. Very painful. So now I know that something is wrong with the design of this brace for a person having arthritic thumbs. Even without the arthritis problem, (5) the interior structure for the thumb was rough and bulky. Uncomfortable. Didn't send this brace back, of course, since I butchered it during a three week process of trying to make it work. However, the hand rehab therapist gave me a choice of two, and I am now very comfortable with a brace by Sammons Preston. Amazon carries some of their products. Very comfortable design, well padded, and nice velco straps that I can handle without using teeth.
