Medical Offices try to go electronic but for legal reasons have to have paper records still
EMRs were to be the deathnell of hard charts. But there's always something wrong with the "computer". Its imperative if doctors want to get paid they need a paper trail on the patient charts of who they talked to, who authorized the visit, the date and time the conversation took place, the name of the person and their id number. You write that on the hard copy of the patient's paperwork which goes into the hard chart so if there is ANY LEGAL ramifications the office practice, and the doctor, are fully covered. There is no EMR today that has the ability to capture what a pen on paper can do at the time of a call with a patient, an insurance rep, a pharmacist who might be denying coverage for the way the RX was worded while the patient is seizing or having heart pains. You want their names, the company they work for. At what location. You write it in the patient's hard chart. You use the 2 prongs to attach it to the patient's Tyvek Classification folder after date stamping it and signing it. This way you present it to the Doctor who has everything they need right in front of them instead of having to go through 8 or 10 different screens of information that might not be readily available and then they made bad decisions. Hard charts are old school "thinking" but they save professionals every time a question comes up. Electronic records can be changed, can be stolen by angry employees, can be hacked by outside forces so everyone one better have an insurance policy in place for their servers against cyber. Hard charts would be the only way to recover from a hack to make sure the information hadn't been changed or shuffled within each patient's/client's charts.Tyvek's are sturdy enough to be re-used 2-3 times before you have to buy new ones - shiny and ready to serve their purpose. Saving lives of both patients/clients/doctors/professionals.

These are great. I run a private practice and my clients are less tech savy so I use paper for my documents with them. These folders have the perfect number of dividers, are very thick and strong, and hold up well over time. 5 stars from me.
