Hi does this have diamonds on both sides and edges

The sharpener is diamond coated steel. The instructions are to use the flat side and the edges to sharpen your blade.

Should I use water when sharpening ?

I do not use water but I do clean the blades I am sharpening otherwise the residue from the plants clogs the sharpening tool.

What are the dimensions: overall length and, width at tip and width at middle (widest)?

Length 11cm, width at middle 2cm, width at end 7 cm. I find it very effective and easy to use.

can this item be used to sharpen a billhook?

I've used the sharpener to sharpen various other blades, so yes it can be used to sharpen other tools. But nb. it is quite small, so I'm not sure if you'd want to use it to sharpen a large blade. The sharpener is only 11cm long by 2cm wide and is flat, so a large blade may require a larger sharpener (not sure how large a billhook is). The dimensions given under product details for the sharpener I think relate to the package as they give much larger dimensions - but the actual sharpener itself is only 11cm x 2cm and flat.
