Doe this have the holes where the ball sits allowing a tee either side?

No ,the holes are further back.i do not find that the mirror moves.

Can you use it on a carpet?

Yes! I use it regularly on the carpet. I'm lucky to have a green flecked carpet strip (15ft) which is perfect for my Spot Putting style, which is complimented by using the alignment line aids built into the Longridge Tour Mirror. I also use it with a Visio Putting Mat and also a Semlos Multipurpose Laser Level which has a Measuring Tape so I set my distances easily. My putting stats have improved considerably since using all three together.

How thick is the mirror in mm?

The whole thing is approx 2/3mm thick. It is not a glass mirror but highly defective plastic . Light and easy to see if your eyes are over the ball.
