And that it's better to have the smaller one vs. a larger one. You don't want it slipping around. I, too, have a large, 4E shoe and the medium fits fine. **Q: Is there a Left and Right ? or are they somehow interchangeable** A: Interchangeable, and the elevation can be adjusted if needed by removing one of the two lifts. **Q: Can it be worn without a shoe or barefoot?** A: No this product cannot be worn without a shoe. Its' purpose is to keep your body in alignment while wearing the post surgical boot. I was able to wear it on a lace up sandal but is more supportive with a tennis shoe. The combined height of your shoe with the Even-Up allows you to walk a normal gait wearing the post op boot. I attempted to walk without the Even Up when I was able to start walking in the boot but because of the height difference I limped along which threw out my back and hips. It is quick and easy to take it on and off to switch to an other shoe if you need to. **Q: Will this work with Crocs?** A: I did not use with Crocs. The way the Evenup is made will not work. It must be a shoe that is made higher across the heel. I used tennis shoes as my doctor recommended for the support of my "good" foot during this time.
