After blaming my printer for not printing correctly on my last ordered forms, I ordered this brand. I am so glad I spent the extra money because it was worth every penny. My last forms would not align properly and half were wasted due to this. These firms are accurate and line up perfectly. DO NOT BUY ANY OTHER BRAND!!! There is a reason these are more expensive. You get what you pay for! Cheaper forms are not worth the aggravation!!.

As an eye doctor I tend to keep buying products from the same vendors year after year. But last month I thought, "I bet I can get these products thru Amazon." So I ordered CMS forms and the quality is the same as where I was ordering from before, and the price is less than half. After having done this I thought abt printer cartridges. I had developed a loyalty to Staples, but thru Amazon toner carts for my Brother printer were >$15 less each, and with 3 colors+black, that's abt $60/refill I save thru Amazon. So my new, belated paradigm is to shop Amazon first.

Having decades of experience with CMS forms, I can tell you these are the most accurate and consistent forms I have ever purchased.Wish I had found Compuchecks sooner. Until these I used to have to re-code the claim form definitions every new package to keep data aligned correctly in boxes.Thank you Compuchecks for an awesome product. I have not had to re-code the claim forms even after buying an entire new box.

They are great! You won't be disappointed!!! We are a busy chiropractic office and have been ordering from other professional distributors over the past 17 years. Wish I would have found these earlier. Could have saved a lot of money. Fast shipping and the forms are perfect. Just as good or better then professional distributor. Very happy!

It is what it is at a reasonable price. I wish I had brought less as the standard is changing later this year. I do most of my mental health billing for psychotherapists through the electronic method. But BB Medical Billing LLC (a little advertising is nice) submits to all insurance companies, electronic or paper. I'll keep up with the standards and hope this company has the new forms by October.

I have been ordering forms from you for about a year and they're always delivered on time or earlier. The forms are good quality and we never have any issues. The price is very competitive.

Came as advertised. The image online is what we received; however, when compared to the form that we had left, the red is more of a faded red vs "red". Almost as if there were ran from a copier.
