Do you have to drill holes on the speaker to attach it to the mounting plate?

Not necessarily. Long slot in the plate can be used for single-bolt attachment to speakers with keyhole mount.

Is there actually anything to hang the speaker on this thing?

No. These just provide a plate you can use to attach the speakers to. Their assumption is (I guess) that you're willing to put screws into the back of your speaker, or that the speaker already has some sort of factory installed mount. In my case with the Polk FXi-A6's, they have a screw hole factory mount so I had to jury-rig attaching to the mounts. I chose not to use these. They stick out too far for my installation and I just didn't like having to jury-rig the speakers to the mount.

Can the speaker mount plate be used as a shelf to hold a small speaker?

No, it won't move to the required angle for mounting that. However you can screw the speaker to the mounting plate and secure it that way.
