Q: Is the handle strong enough to hold 21 lbs?

It depends upon the material the handle is screwed into. If it's a strong connection, it should easily hold more than 20 pounds.

Q: What is the length, center-to-center , of this handle? I need to replace a handle on my Taylor case, which is 5 1/2 inches center-to-center

Not sure which centers you need. Here are the distances between the center points of the 4 holes: 1 7/16 inches between the 2 holes on each side of the handle, 2 13/16 inches separating the inner holes, and 5 11/16 inches separating the outer holes. Since the handle on my horn case was attached with rivets, which were hard to extract without damaging the holes, I ended up screwing the handle into new holes. My case is plywood under the covering, so this worked for me. Hope this helps.

I’m not sure what kind of plastic or metal. It was very sturdy, looked nice.
