Great product (re-evaluated)
I first want to say I trashed this product during my first review, however, I've learned the reason why it didn't work at first. Maybe it'll help you.I brought this product because my CPAP was making a horrible whistle noise despite all my efforts (which included new mask, hose, and locations for equipment). I've learned that the issue was my ramping was starting off too low which did not give me enough pressure which created the whistling. I changed the settings to a more suitable starting point. This allowed a more quieter startup.I noticed that even with the modifications there were small whistling here and there along with a noticeable blowing noise. Once I added the muffler it literally made all the noises disappear. The only time I hear something now is when something is obviously wrong (eg: the mask not being fitted properly).Overall a great product if you're like me and want complete silence from your CPAP equipment. Just know it comes with caveats.

Helps, but not perfect for DreamStation.
From the research I did, it seemed this device could be used with almost any CPAP machine so I'm giving it a try. My situation is that I have a Philips Respironics DreamStation with optional humidifier. To me, that unit is super quiet with the humidifier unit attached (with or without humidification being turned on). But, without the humidifier unit attached it is quite loud to me. I hear a lot more motor/fan noise and wind rushing noise in my mask without the humidifier unit attached. I need to travel quite a lot and prefer not to bring the humidifier with me to save space and weight. So, I'm giving the4 Q-Tube a try to see if it helps. I've used it for a few nights now. To me, the result is somewhere between the DreamStation without the humidifier and with...maybe right in the middle being subjective. With the Q-tube, there is definitely less noise when using the DreamStation without the humidifier. But, it's not as good as using the DreamStation with humidifier and no Q-tube. I can sleep with it and will plan to go this route for travel until I consider buying a dedicated travel CPAP, but that is a personal opinion. I hope this helps someone in a similar position.

It actually works
Like others, I have a love-hate relationship with my HDM Z1. It's nice to just have it packed up and ready to go, and no distilled water needed, but for quite a while I was wearing earplugs to keep it's excess noise from keeping me awake. That go old and it didn't impress my wife. I ordered new heat/humidity carts and this came up as an "also purchased" so I decided it was worth $25 to see if it helped. I tried it out for the first time tonight and while no, it's not as quiet as my ResMed unit, it's not far off. I'm pleasantly surprised. My one complaint is this: If this is made by HDM, then HDM needs to just add it at cost to the (fairly high) cost of the Z1 itself. To leave it out of the package is sort of like selling someone a new car and then selling them the muffler after they drive the car off the lot and get fifty letters from the neighbors complaining that the car is too loud. No, my neighbors can't hear my Z1, but until I bought this, I am kind of surprised by that fact. Good product. Worth $25. HDM...think about my advice.

Does NOT muffle anything - I measured.
I testedusing noise measurement instruments on three different CPAP models. NO difference with or without this gadget. It does not have a muffler construction either. It does have super difficult to access chamber lined with an open cell foam (spare foam included)= a perfect bacteria breading ground. The diameter of the enclosed rubber connector and gadget diameters are not standard 15 mm so they require a fight with elements to make connections but it can be done. Almost good idea but on needs a real engineer to make a real muffler. I does nothing for the noise at the CPAP machine and nothing for the mask noise.

It worked for me.
I did notice the difference. I have a travel cpap that was pretty noisy and sometimes had trouble sleeping when I was traveling. I found this muffler and when I used it a few months ago on a cruise, it worked perfect. I did notice the difference.

Does adequate job but room for improvement
Does it work to muffle the sound? Yes. Does it muffle it enough to equal a standard sized CPAP machine? Definitely not. Will I continue to use it? For sure! The unit does help to muffle the sound, however I can still hear a distinct whine through the hose when I'm breathing and this causes me to wake up more often. I can't fully attribute waking up to just the sound because I am using this muffler with my travel CPAP and I am sleeping in unfamiliar places and beds, so some of my restlessness needs to be attributed to that factor.I can say definitively that my wife appreciates the muffler because she can't hear the machine as well as before.I am giving this unit 3 stars because it works, however I think future versions have a lot of room for improvement. Overall I rate is as a "buy."

The savior of noisy travel CPAP machines
I recently bought a HDM Z1 Travel CPAP for work trips. When received, like many reviews have noted, it was obnoxiously loud through the air tubing, specifically the high-pitched frequency of the little machine's fan. I bought this muffler to help eliminate the noise, or was going to return the machine if it didn't. I was not able to sleep with the noise. THANKFULLY, this muffler worked wonders, and I was able to sleep soundly the last couple nights without complaint!
