If i were to hang upside down with the vest, would it fall off?

It shouldn't if you have the side straps attached correctly

Would this vest be ideal for a woman to use?

I would say it is not ideal for a women. The vest does shift and bounce some, so it may not be comfortable on your chest. I would recommend a backpack style, or weighted belt.

Is the vest washable?

What is the weight of the Vest without any 3lbs. weights installed?
