i don't think you can hook it on a unicycle. no where to hook it.

Does this work on a bike with a gear system?

I would think it might however we put it on our grandsons bike and it is just a single speed. It would depend on if your sproket got very big. It rubs on the spokes of the back wheel is how ot works. It can be put on either side so I might tend to believe it would.
