Q: Will the slots hold CR123 batteries?

yes, the slots made by the elastic band.

Q: Does this bag have velco on the back to aid in attachnent to the helmet?

No. But the Tactical helmet counterweight bag can be attached to the helmet by screws on the helmet rail. Simply unscrew the screws at the rear of the helmet rail, and then attach the left and right ends of the helmet weight bag to the helmet rail, finally tighten the screws. Simple to install, firmer than Velcro

Q: Is this compatible for ach or ech?

It will fit on both a ACH and a OPSCORE helmet. Just helmet to helmet with current screws.
