Is there a + on the bottle by the name? That would be the new formula and it is terrible. I'm looking for the original formula (without the +)

I have never seen a + by the name on the Dram. Different bottle. However, Clinique DOES make two types of the product - one is called Lotion and the other is another name (forget which) and they are supposed to be for different types of skin. I don't find much difference between the two - and use either/or. They both work wonderfully.

Is this a body lotion and not a facial lotion moisturizer?

I use it on my face only, and it's marketed as a facial lotion. There's nothing to keep you from using it as a body lotion, of course, but you certainly can find much less expensive lotions for that purpose. I doubt you'd want to spend $30+ on just 4 ounces of lotion that you plan to use on your body. I, personally, use Lubriderm on the rest of me!
