I got the standard one. It is not firm enough. Squashes nearly flat when I use it. A rolled up towel works better.

How firm is the firm? I like a really firm support and usually everything is too soft...

This is my experience using the McKenzie Lumbar Roll on my office chair (backrest having some built-in lumbar curve) and leaning back with 170 lbs. body weight: Standard Density model compresses from 4.75" diameter down to 2.5" to 3". Firm Density model compresses from 4.75" diameter down to 3.5" to 4". The Standard Density is much softer and works well with a chair backrest that already has some lumbar curve. The Firm Density pushed too much from the backrest against my spine.

Has anyone ever used this roll in the lower back to sleep on?

The roll would be too dense to sleep on. If you need some low back support to sleep, you can try what my physical therapist taught me: Take a bath towel, lie it flat, and then place an ordinary belt lengthwise on the long edge of the towel. Roll the belt up inside the towel. You can then belt the towel around your waist, and you will have support whether you lie on your back or your sides. Sounds crazy but it works!

Where do you position it?

My orthopedist instructed me to place the roll at the waistline, not lower, where it would push the waist vertebrae into the wrong position. The back is supposed to have a slight concave curve at the waistline. Wearing the roll lower creates more of a straight spine from top to bottom. Not good. BTW, I have used the McKenzie roll since 1976. For someone with a bad back it makes long car/plane/bus rides totally doable. There is no roll out there that could perform better, I'm sure!
