Appears to be a genuine OEM. I made the mistake (once) of buying and off brand. I won't make that mistake again.

Worth the money
The low priced substitutes are not good. Although item is not cheap, it does hold a charge for weeks and does the job. The fit is exact as you would expect.

So far seems to be a legitimate Milwaukee battery.
While I found this to be a bit overpriced, I have a very nice drill set that these batteries are no longer being made for. One battery had gone bad, the other isn't holding a charge for as long as it once did and I hated the idea of throwing away perfectly good tools because I had no battery. They're running like new again. If this should not last long, I will update my review.
