Is it to apply to the nail bed, not on the nail?. My nail fell off as it was separated from bed.

Yes it is for the nail bed. However look at the label . They have changed the ingredients and it now has 99 % alcohol and 1 % tolnaftate . It smells of just alcohol. The previous formula did work . I do not know about current formula.

Why does the back of box say not effective on scalp or nails on back of box? the picture on box shows a toe with a drip of the solution!

The FDA makes clear guidelines on what OTC active ingredients can state about product effectiveness. The illustration shows brushing around the nail bed and skin. This product works on the cuticle area and under the nail bed. There is no clinical evidence that the active ingredient does penetrate through the nail. Scalp application is not products intention or for use.

I have orange fungus growing on my toenails after swimming in a lake. Anyone have success with curing orange toenail fungus with this stuff?

I don't know about orange fungus , however they changed the formula . Since then it hasn't cured anything .
