Very strong suction -- the second to lowest setting was plenty for me. The tube is a little awkward and I feel like the tip could be flared a little more for those of us that have large tonsils. My tonsil crypts are pretty big and the tube sucked itself right into some of the holes. But overall, I like the tonsil vacuum and am hopeful that I'll get used to using it.
