what size would fit a 18" neck?

I' wear a size 16 neck size and the large fits perfect. I'd suggest the extra large.

Yes, I've used it daily for the past ten months. I put it on for about ten to fifteen minutes. My internist, neurosurgeon and local chiropractor said it's fine to use.
