how exactly is the "locking mechanism" for the adjustable ankle-holding knob lock in? adjustment knob does not lock into place on my table.

The lever on the ankle lock has different settings. You pull the lever inward after setting your ankles in between the two foam sets. It will automatically at the desired ankle size. I usually put the small silver locking pin through the holes at the bottom of the locking lever. It stays in its place after that.

Can you invert to 180 degrees?

No, the Gravity device has a bar in the back for safety reasons that will prevent the 180 degree (straight up/down). With the bar, your at about 175 degrees. The bar is not locked into the frame, so if you want you can take it out. Although I would not recommend doing that, as you might get stuck upside down. Its a great product, I use mine every day for about 10 minutes in the morning and 10 minutes at night.
