I am currently installing the drain however should the rubber sleeve threaded coupling be glued to the metal drain outlet, it seems to just slip off?

No, don’t glue the sleeve in place. The mortar bed and tile will hold the drain and insert in place

Has anyone had this product for an extended period of time? Any failure or quality issues?
