does the magnetic name tag come with the magnet as well or are they just the nametage with the magnetic backing and no way to hold them on ?? -

Yes they come with the magnet too. Magnets are strong. I've had no trouble with the name tag coming off at all.

I will be buying from Amazon- there does not appear any way to request a name to be inscribed- how does this work?

Once you get to the page when you submit your order you will be able to inscribe there in the spot that says gift

Can the name (first line) be bigger and bold than the subsequent lines?

The size of the characters per line, depends on the number of characters. With less characters, the words will appear larger. This holds true for each line. You are able to enter in all of your information and it will show you a "proof" immediately, which you can go back and tweak until its perfect.
