It's neither pepper, nor spearmint - it's in-between; flavor is not that strong.

My husband uses a CPAP. How long till the relief from the Xylimelts worked?

Mary, my husband uses a C-Pap nightly and has severe dry mouth also, due to meds. The xylimelts have been 100% effective for giving him the moisture he needs.
