Q: How long can you wear each mouth guard before replacing it?

Depends the use. I wear it every night for a long time and haven't replaced it yet

Q: does it cover the far back molars as well as all the teeth ?

It covers but it’s not comfortable I tried once and I was a bad experience

Q: Can you use this if you have an implant?

I guess you can. I dont have an implant, helps with grinding while I sleep. If your implant is too sensitive and you feel pain it might help then again it might not. Its a gamble 50/50
