The zoo med bulb i have says 22"/559mm reptisun 5.0 uvb f24t5/rept. is this bulb the correct replacement?

Yes this would be the correct bulb. If the hood itself is 24 inches long, it holds the 22 inch bulb.

Is this the lamp and the bulb?

This is a lap only. You would need to buy the fixture separately. You will find that most lamps and fixtures are sold separately

How long should these last?

The older style, curly, bulbs were supposed to be effective for no more than 6 months. These straight bulbs should provide proper amounts of uva and uvb for up to one year. I wouldn't use them longer than that or the reptile might suffer from MBD. Even if the bulb still provides light the useful feature will be spent by then. I use two inline fixtures and change one every year alternating each 6 months.
