Can you just paint this on to help a patch of new concrete adhere to old concrete? Or do you have to make a slurry?

Yes. Brush it on like paint and let it dry then apply the new concrete. You can even add it right to the concrete mix too.

What is the coverage for the quart?

This has a coverage of about 14-15 sq. ft for a quart.

I have a bird bath pedestal that broke on top and is not level, can I use this to level it out and put a layer of cement over it?

It’s a liquid that you can apply with a paint brush. You then let it dry and it’s helps new concrete bond to existing concrete. Since it has the consistency of paint, it cannot be built up to make it higher on one side than the other. The concrete mix could problably be layered in a way to fix minor leveling issues - not the bonding agent. Think of the bonding agent as glue. It just makes sure that any new concrete will bond properly to any old concrete. For that purpose it works great.
