Q: How do these work if you have to have to wear a comm headset?

I really don't know. I suppose you are protecting your ear from your comm headset and I thought you could regulate the volume of the comm headset. Anyway, I just got this Ear protector about two weeks ago and I use it for orchestra rehearsals where I could not presonally regulate the outside noise. There are still much information I have to learn from this which unfortunately not provided by the company but I saw a helpful 20 minute video from an audiologist on YouTube which you might get some information - I am sorry I cant locate it but perhaps if you call Etymotic Research you can get the link . I plan on doing it too. Good luck.

Q: Can you swap the silicone and foam ear tips for custom molded tips later on?

The tips come off, so if you can have customs made to fit the unit itself – which seems to be a fairly standard fitting – I would assume you can do that, although the custom maker is best suited to answer that question. As a side note, in my use of these, I find the foam tips are substantially more effective than the silicone ones. They just block more noise and fill the ear canal better.
