how does it compare in effectiveness to Malaseb?

I have never used Malaseb. But I actually used this shampoo on myself (I don't have any pets). I used it for candida overgrowth and it was effective. It also left my very long hair (past my butt) very soft and manageable.

How many times should I bathe my Guinea pig and soni leave it for 10 minutes as well?

Yes leave it in for 5 mins or so for the Miconazole to to do its job

How often do you have to bathe your cat for this to work on ringworm? Did you have to leave it on for the full 10 minutes each time?

My dogs are 15 pounds each, so the size is probably similar to your cat. I bathed mine once a week, and yes for my non squirmy dogs I left it on the whole 10 minutes.

Does this work as a lice treatment for Guinea Pigs?

No ivermectin is used to treat guinea pig lice. This is good for a fungal treatment or preventative.
