What is the actual max output? Description says 5v/2.5v, but picture shows 5v/3v label.

max output is 5V/2.5A, after check we didn't see any picture shows 5V/3A

The picture shows 2.5a output the specs say 2a which is it I need a 2.5a output?

Mine is 2.5A printed. Most dashcams don't consume more than 1A, so don't worry if is 2 or 2.5A output rating.

is this the right kit for Garmin 67W dashcam

This might work but I bought a dedicated cable made by Garmin which is compatible to their products. Search for Garmin 010-12530-03 Parking Mode Cable. This is quality made and cable are safe unlike other cheaply made power cable.
