Q: Can this be fitted on a cot bed?

It fits perfectly on my nieces single bed so I doubt it would fit on a cot bed. The way it holds is it tucks around and under the mattress so a cot bed mattress will be too small?

Q: Where do the two side poles go? We have had to walk away from this several times and it's driving us mad! We can't find the holes / openings

there are two tiny holes in the tent, at the bottom of end archway, you'll find a small slit in the fabric.
