Yes there is a short hose that clips on to the main hose.. hint for machine I use frozen water bottles instead of ice works great

Will this pad work for your back?

Yes, the band should be long enough to wrap around your waist and secure the pad.

What pad would you use for shoulders and neck?

There is a shoulder pad. However, you could use the universal pad for shoulders and neck as well.

I have to have rotator cuff surgery. If I get this universal pad, are there instructions or diagram as to how to use it and straps?

Yes. I received instruction for each type of pad. You don't have to put them on as shown, and can place the pad where it is best for you. Watch that it doesn't get too cold and give you "freezer burn". I'm not kidding!
